1.Pre-Employment Background Check Contract

This is a contract that must be signed by the client before Hons Investigations can begin processing pre-employment background checks for your company. Please print the form, sign and return to us.

2.Pre-Employment Employee Authorization/Release

This is the form that you or the employee must complete and sign. You may then fax us this sheet with the below Fax Forwarding & Request Form, or you can go our ONLINE SEARCH ORDER FORM and forward the information to us electronically.

3.Pre-Employment Check Fax Cover Sheet

This is the request form that must be faxed to us with the completed employee form. On this form you will check which searches you would like us to conduct, as well as provide your contact information.

The FCRA requires that an employee be provided with these rights.

B. EMPLOYMENT APPLICATION

Download this form and complete and return to Hons Investigations to apply for a position with our company.
